All fees are to be paid at time of service.

If you plan to seek third party reimbursement, it is advised that you contact your insurance company prior to the first session and ask the following questions:
- Is my provider in-network?
- What percentage of my bill will be paid for services obtained from an
out-of-network provider? - How many sessions will be covered per calendar year?
- What is the coverage for CPT code 90834 45-60 minute session?
*Please note: There is a $35 fee for all returned checks without exception.
*Please Note: Missed appointments will also incur a $25 fee.
Your insurance will be billed for your session, however if the deductible has not been met you will be responsible for the cost of the session. Your co-pay amount will be due at time of service.
